Animal Control Forms

Leon County Animal Control service forms available via the web page for Leon County residents.  

There are three forms that are routinely used by Leon County Animal Control to help service the citizens of Leon County.  There forms are: Trap Request and Agreement Form, Affidavit of Complaint Form, and Petition for Classification of a Dangerous or Aggressive Animal Form. A brief explanation for the use and completion of each form and a link to the actual form is listed below. Please read about the use of the form and then click on the link to the form.

Trap Request and Agreement Form:
This form is used to request Leon County Animal Control to set a humane live trap for a nuisance or stray dog or cat. Please fill in the areas in Bold letters. Either mail or drop off the original form to Animal Control at the address listed or fax to (850) 606-5401.

Affidavit of Complaint Form:
This form is used to file a formal sworn complaint against a pet owner for violating the County Animal Control Ordinance. Fill in all areas that relate to your complaint, be specific on details, include the date and time of violation, the owner name and address if known, and description of the animal in question. Then the form needs to be signed before a Notary Public. Mail original form to Animal Control at the address listed. No faxes. The complaint will then be investigated by an Animal Control Officer, and if valid, a citation may be issued. Affidavit of Complaint

Petition for Classification of a Dangerous or Aggressive Animal Form:
This form is used to file a formal sworn complaint on a owner's pet that has shown dangerous or aggressive behavior against a human or animal. Fill in the first page with the pet owner's information, a description of the animal(s) in question, and check the appropriate violation. The second page is for a narrative of the incident in question, any history on the animal, location of the incident, specific date and time of the incident, and any witnesses. Then the form needs to be signed before a Notary Public. Mail original form to Animal Control at the address listed. No faxes. The complaint will then be investigated by Leon County Animal Control.
