OUR MISSION

The mission of the Primary Health Care program is to improve the health of citizens by providing quality and cost-effective health services through collaborative community partnerships.

WHO WE ARE

Leon County partners with local health care providers to implement and fund state-mandated program such as the Health Care Responsibility Act (HCRA) as well as Board-established programs such as the Leon County Health Care Program that ensures uninsured and underinsured residents have access to comprehensive health services including primary care, dental care, mental health, specialty care, and pharmaceutical services.

Bond Community Health Center accepts Medicaid/Medicare and private insurance. The health center also provides healthcare on a sliding scale with verification of income. Please bring proof of your insurance or documentation of your income to your appointment. Walk-ins are welcomed.

Neighborhood Medical Center (NMC) is a community health center dedicated to providing Leon County residents and the surrounding counties with high-quality comprehensive primary healthcare. Free or reduced healthcare is provided for eligible Leon County patients based on verification of income. Please call to schedule an appointment.

  The We Care Network provides free, donated specialty medical care for patients referred by their primary care physicians. Patients must be uninsured and not eligible for any other government program. Patients must be Leon County residents and meet income guidelines based on the Federal Poverty Guidelines.

The FAMU Pharmacy provides pharmaceuticals to eligible patients with written prescriptions from a CareNet provider. Medications are priced on a sliding scale based on the patient's ability to pay. Income verification must be provided. The Pharmacy is staffed by FAMU College of Pharmacy and Pharmaceutical Sciences.

Services Provided: Family Planning, Immunizations, T.B., Sexually Transmitted Disease (STD), HIV/AIDS testing, treatment and counseling.

Through Florida KidCare, the state of Florida offers health insurance for children from birth through age 18, even if one or both parents are working. It includes four different parts. When you apply for coverage, Florida KidCare will check which part your child may qualify for based on age and family income. You may apply online.

 

MENTAL HEALTH

Mental health is critical to the well-being of all individuals. The Leon County community has a wide variety of mental health care services and programs that range from crisis stabilization and outpatient therapy and treatments (i.e. psychologists, clinical therapist, etc.). If you our someone you know are in crisis, the referrals below are available 24/7 and with connecting you to mental health services.

The 211 helpline provides free access to information, referrals, and confidential telephone counseling services utilizing a comprehensive referral database with more than 2,200 human service agencies and mental health providers in the Big Bend area.

The 988 Suicide & Crisis Lifeline provides 24/7 free and confidential support to individuals in suicidal crisis or emotional distress and their families through a national network of local crisis centers.

Law Enforcement Mental Health Response

The Consolidated Dispatch Agency (CDA) coordinates with Mental Health Responders from the Leon County Sheriff’s Office and the City of Tallahassee’s Emergency Assessment Mobile Unit (TEAM) to provide crisis intervention and assessments for individuals experiencing non-violent mental or behavioral crisis, and emergency behavioral health services in accordance with the Baker Act.

The Apalachee Center’s Mobile Response Team is a 24/7 on-call crisis intervention service providing emergency behavioral health assessments and diversion alternatives to inpatient treatment. Services are provided through phone consultations and on-site response by Mobile Response Team clinicians.

The Veterans Crisis Line provides 24/7 free and confidential support for veterans and service members in crisis, even if not enrolled in VA benefits or health care. The Veterans Crisis Line serves veterans, military service members, National Guard and Reserve members, and those who support them.
